Childhood cavities can progress quickly because baby teeth are smaller and their protective enamel and dentine are thinner than adult teeth. A child may have little pain at first, then suddenly develop a toothache, swelling or difficulty eating.

The good news is that many cavities are preventable, and early decay is much simpler to manage than an infected tooth.

Parent checklist: look for white chalky areas, brown or black spots, food trapping, sensitivity, pain while chewing, bad breath or swelling. Do not wait for severe pain before booking an examination.

How Does a Cavity Start?

Oral bacteria use sugars and fermentable carbohydrates from food and drinks to produce acids. Repeated acid attacks remove minerals from enamel. If demineralisation continues faster than the tooth can repair itself, a cavity forms.

Early loss of a baby tooth can also allow neighbouring teeth to drift, potentially reducing space for the permanent successor.

How Are Cavities in Children Treated?

Treatment depends on how deep the decay is and whether the dental nerve is involved.

Early non-cavitated decay

Very early enamel changes may sometimes be managed with improved plaque control, fluoride and dietary changes while the tooth is monitored.

Small to moderate cavity

The decayed area is removed and restored with a suitable filling material.

Deep decay near or into the nerve

A pulp treatment such as pulpotomy may be needed for selected baby teeth, often followed by a stronger restoration such as a crown.

Severely infected or non-restorable tooth

Extraction may be necessary. If a baby tooth is lost too early, the dentist may assess whether a space maintainer is needed.

What Are Fissure Sealants?

Sealants are thin protective coatings placed in deep grooves of back teeth. They create a smoother surface that is easier to keep clean and can reduce decay risk in susceptible molars.

What Role Does Fluoride Play?

Fluoride helps strengthen enamel and supports remineralisation. The amount of toothpaste should be appropriate for the child’s age, and young children should be supervised so they do not swallow large amounts.

Does a Child Need a Filling If the Tooth Will Fall Out Anyway?

It depends on how soon the tooth is expected to exfoliate, how deep the cavity is, symptoms, infection risk and the role of that tooth in maintaining space. A tooth with years left before natural loss may need treatment even though it is a baby tooth.

When Is a Child’s Toothache Urgent?

Seek prompt dental care for facial swelling, fever, pus, severe night pain, difficulty opening the mouth, trouble swallowing, or a child who is unable to eat or sleep because of dental pain.

How Parents Can Prevent Cavities

  • Brush twice daily with age-appropriate fluoride toothpaste
  • Help or supervise brushing until the child has reliable technique
  • Reduce the frequency of sugary snacks and drinks
  • Avoid putting a child to sleep with sweetened drinks
  • Ask about fissure sealants for cavity-prone molars
  • Keep dental reviews at intervals based on the child’s risk
